Introduction: It has been a hotly debated issue whether to send all major head and neck cancer
patients having free flap reconstructive surgery to an Intensive Care Unit (ICU) postoperatively.
A previous audit, from our unit, concluded that the routine use of a specialist head
and neck ward (SHNW) was cost effective without having an adverse affect on care (BJOMS
2007). A decision whether these patients should go to ITU or SHNW is dependant on
the assessment of an experienced anaesthetist. A change in anaesthetist prompted us
to reaudit our current practice.
